Nomination procedures for Lifetime Artistic Achievement Awards

Assessment criteria
- The excellence of the nominee’s work, covering all aspects of artistic quality, creativity, innovation, and technical mastery
- The quality and extent of the candidate’s lifetime contribution to the development of the performing arts in Canada and to the cultural life of Canada
Documentation
Each nomination must be proposed by one principal nominator.
Nominators must complete the nomination form for the Lifetime Artistic Achievement Award, which includes the following items:
- The coordinates of the nominator (name, address, telephone and fax numbers, and e-mail address) and the name of the nominee. If known, please include the telephone number and email address of the nominee.
- A declaration confirming that he/she and the nominee meet the eligibility criteria described in this section and that the information contained in the nomination form is accurate. If the form is being mailed or faxed, a nominator signature is required.
- A statement or letter describing why the nominee should receive a Governor General’s Performing Arts Award for lifetime artistic achievement. Focus on the nominee’s significant national and international achievements and how the nominee has contributed to the development of the performing arts in Canada and to the cultural life of Canada.
- A current biography of the nominee, with emphasis on his or her achievements in the performing arts.
The official nomination form for the Lifetime Artistic Achievement Award must be used to provide the required information. The Foundation will not accept bound materials (e.g., stapled, Cerlox or spiral-bound) and is not responsible for the loss or damage, whatever the cause, of support material. Any support materials sent to the Foundation will not be returned.
